For all new students and incoming Kindergarten students, please fill out the following paperwork listed below.  All of these items must be present in order to register your child and make a class assignment.

*We like to give our teachers one full school day notice to prepare for a new student.  Once the paperwork is fully turned in, your student will be able to start on the following day or as directed.

In order to register your child, please bring the following:

1. Registration Form Print, complete and sign.

2. Guardianship Status Form Print, complete and sign.

3.  Demographic Form (Proof of Residency) (Provide one document from Column A and one document from Column B and fill out the right side of the paper) Print, complete and sign.

4. Homeless Assistance Act - McKinney Vento Print, complete and sign

5. Record of Special Services and Health Information - Services your child may be receiving or has received.

6. Utah School Immunization Form Print, complete and sign.

6. Media Opt Out Form - If you would like to opt out of your child's photo being taken.

To complete the enrollment process, we will also need:

• Original birth Certificate (We will take a copy for our records)

• Original immunization records

• Guardian photo I.D.

• Two proofs of residency
